Role Overview & Responsibilities

The Social Work team within the High Wycombe AMHT is part of the renewed commitment by Oxford Health NHS Foundation Trust and Buckinghamshire Council to continue working in partnership. Together we deliver mental health services for the residents of Buckinghamshire and those placed out of area for whom we are the responsible body.

You will be supported by an Assistant Team Manager and work with social care and health colleagues to deliver excellent outcomes for adults who need our support and their families.

You will maintain a strong Social Work identity and act as an ambassador for Social Work while working collaboratively with secondary mental health colleagues both within the AMHT and across the wider Trust.

You will use relationship based Social Work skills to understand the needs of this working age adult service user group and work collaboratively to create care and support plans that reflect strengths-based and person-centered outcomes.

The post-holder will have strong Social Work values and a commitment to improving the lives of adults with complex health and social care needs and their carers.
