Role Overview & Responsibilities

We are looking to recruit two motivated and enthusiastic assoicate practitioners to join the Older Adult community mental health team (OA CMHT). The service operates 5 days a week throughout the year, excluding bank holidays from 9am to 5pm. We provide ongoing support including holistic assessments, and person centred recovery treatment plans for older adults experiencing mental health difficulties or dementia.

Whilst we support people with a range of mental health needs such as anxiety and depression or psychosis. We also offer support in the assessment and management of dementia, including monitoring the titration of dementia and routine review of dementia medication.

We are looking for a motivated and experienced support worker to join our team of staff as an associate practitioner, supporting individuals in the community to live well and as independently as possible.

Whilst this post is advertised for a band 4, we will consider applicants at a band 3, who are motivated to develop their career by completing any required further training to enable them to progress to a band 4.
