Eden Brown Synergy are actively recruiting for an experienced CHC RNLD Nurse Assessor for a long-term contract based in London. For the past 30 years, our client have prided themselves on partnering with continuing healthcare and social care teams to deliver independent, standardised and improved pathways.

EBS are looking for an experienced RNLD Nurse Assessor who holds a background within continuing healthcare that is either immediately available or available within a 2-4 week period.

Your responsibilities for this role include:

• Travelling to complete face to face visits with patient's four times per week as well as being office based one day per week.
• Undertaking person-centred assessments to CHC eligibility.
• Ensure quality of care and contribute to that culture to achieve results.
