  • Are you an experienced Care Coordinator?
  • Are you passionate about helping people?
  • Do you hold your NVQ Level 2 or 3 In Health & Social Care?
  • Do you hold a full UK Driving License and have access to your own vehicle?

We are currently recruiting for an experienced Care Coordinator to join a hard working and dynamic team.

As a Care Coordinator your role would include but not be limited to the following:

  • Scheduling weekly care visits for our community based staff
  • Identifying capacity, bidding on and scheduling new care packages
  • Contributing to our On-call / out of hours service
  • Providing excellent levels of customer service
  • Adapting to changes and dealing with pressure in a sometimes fast paced environment
  • Liaising as necessary with service users, next of kin, professional partners
  • Working alongside our Quality Monitoring Officer to arrange new service user assessments and care plan reviews as required
  • Ensuring a high-quality and prompt service at all times

In reward for your hard work you will receive a salary of £20,000 to £21,500 and generous bonus scheme for achieving pre-set targets.
